Fingerprint card

Getting ready to get my fingerprint card done. 2 questions: First, do I fill anything out ahead of time on the top, or do I wait until I'm there? Second, when it says Employer and Address, do I put my regular job or put self and my home address?

Leave it blank but most places around me now print onto their own cards and give you a copy or 2. Its identical as the ones the BATFE sends you minus the pre-typed address.
