I remembered this happening to me on the shoot site. It is in my latest video. Now that I have this on video, I thought it would be a great learning experience, particularly for those new to 1.3 product. I have a theory on what I think this is, I want to first hear what other members think is happening here. If anything it sure illustrates why eye protection is always needed. The video will first play at normal speed then slow speed.

Do these shells have visco on them? My thought was that the extra inch or two of visco in the QM is being launched out from the gasses inside the QM

I'm going to guess that an e-match wire was draped over the top of the tube; or someone tied off the e-match to the inside of the tube with too big a knot; and deflected part of the shell lift charge as it fire out of the tube.
Something had to be over the top of the tube to deflect the charge like that.

If i had to guess, I would say that this was an e fired shell/show and that this shell had a short e-match port near the lift to fire the shell. What you are seeing is the quick-match leader burning off that would have been used to hand fire the shell. Most people that use this e-match port cut off the hand fire leader to prevent this kind of thing from accidentally lighting other effects or starting fires. Aas far as using eyeprotectiion at this distance: . . . . What other personel protective gear should this "cameraman" be wearing?

I'm with playinwithfire on this one. It was a hand lit show. The piece of visco is shooting out. We have all seen what a 2 inch piece does by itself on the ground, it either spins around or shoots off.

I think Northern Sky is right on this one. At first I am thinking it was a piece of the shells tail. The fire trail looking like branching sparks. But a tail would not travel like that before the shell even left the mortar. But what really make it look like a fuse is that last second direction change. This was a hand light display. What we are seeing is most likely the short leader for the electric match port breaking away from the main leader, the quick match burning gave it some thrust. All of these shells had Visco light points. I am not sure I can magnify one of the stills enough to see it. But it would be cool if I could see the fuse at the tip of that fire. This sort of thing gives you another reason to also keep your finale covered.
